How Case Managers Help Clients Access Essential Equipment

At Blue Skies Case Management & Care, our case managers are committed to helping clients get the equipment they need to live fuller lives. Through Immediate Need Assessment reports, coordinating with occupational therapists, and liaising with solicitors for funding, our team facilitates access to life-changing resources.

Client using his off road wheelchair

One of our clients, Aidan, recently experienced this support firsthand. Thanks to his case manager, Catrin, and the help she secured, Aidan was able to enjoy a dog walk with his new off-road wheelchair. This allows him to navigate more challenging terrain, something that would have been impossible without the right support.

Step 1: Immediate Needs Assessment Reports

The process begins with a thorough Immediate Needs Assessment, where the case manager works to understand the client’s lifestyle, needs, and goals. For clients with mobility challenges, this assessment helps identify specific equipment, such as off-road wheelchairs, adaptive aids, or accessible motor vehicles, that could improve their independence. The case managers role is to identify the need, inform the solicitor who will agree funding providing that there’s clinical evidence that it will improve the life of the client

Step 2: Collaborating with Occupational Therapists

Once the need for specific equipment is identified, case managers collaborate with trusted occupational therapists who have the expertise to recommend tailored solutions. These professionals conduct thorough evaluations to ensure the chosen equipment meets the client’s physical and functional needs. In Aidan’s case, the occupational therapist recommended an off-road wheelchair that would best support his outdoor activities. With so many options on the market, having an occupational therapist's guidance is essential to avoid costly mistakes and ensure clients get the right fit for their needs.

Step 3: Securing Funding with Solicitors

Case managers work closely with solicitors, who act on behalf of clients to access funds necessary for their care. By providing the required documentation and advocating for the client’s needs, case managers help ensure clients receive the funding needed for their equipment without unnecessary delays.

Real-Life Impact: Aidan’s Story

For Aidan, this approach made a real difference. After Catrin helped secure his off-road wheelchair, he was finally able to take his dogs out on walks he’d missed doing. With the right support, Aidan now enjoys a renewed sense of freedom and connection with nature, a simple pleasure made possible through careful planning and coordination.

The Role of Case Managers in Empowering Clients

Case managers play a crucial role in empowering clients by helping them access tools that enhance their independence and quality of life. From Immediate Needs Assessments to securing funding and working with specialists, case managers drive these impactful changes.
